📝 Notes

Notes is the agency-wide ledger of visit documentation — every note by every nurse, searchable and reviewable.

Finding notes

The Search & Filter bar works with filter pills:

  • Patient first name / Patient last name / Nurse — click a pill to open a search box for that field; searches apply as you type. Click the pill's × to clear it.
  • After date / Before date — calendar pills that bound the visit date.
  • Service — a view filter that narrows the list to one service type (Respite, Personal Support, Lifeskills, Supported Living, Supported Employment). Because it filters what's shown page by page, the footer tells you exactly how many rows it's hiding.
  • Newest/Oldest first — the sort toggle on the right.

The table shows nurse, patient, service badge, and the visit date (toggle Show Created Date for the entry date too). Notes you've already opened are tinted so review sessions don't lose their place. Pagination supports first/last jumps and typing a page number directly.

Reading a note

Open any note to see the full record: the written note (the AI-generated narrative), health status, behaviors, the goal worked on and its progress, each event with its methods and choices, safety steps, monthly education status, and any additional information.

Creating and editing

  • New Note (sidebar or dashboard) opens the same guided wizard nurses use on mobile — useful for documenting on a nurse's behalf. As an admin you can select any patient in the agency.
  • Notes can be edited after the fact — including regenerating the written note after changing the underlying details.
  • Deleting a note removes it permanently; Noted confirms first.

The written note

The narrative is generated by Noted's AI from the structured details — see AI Note Generation for exactly what it does and never does.
